• Responsibilities of applicants:

To complete the application and supply all information requested.

Attach a copy of a written estimate or itemized bill from their veterinarian to the completed application.  The estimate or bill should be on the veterinarian's letterhead and should include the following:

1. Name of attending veterinarian and facility

2. Applicant's name

3. Dog's name

4. Diagnosis

5. Prognosis

6. Complete breakdown of costs
